Developing a 5 year goal plan with 3 blocks: cognitive, social, and physical.

I have to have three blocks in a 5 year goal plan: cognitive, social/emotional and physical. With three to four activites in each block that would help me to meet my goal and they have to be done in the seasons (winter, summer, and fall). What are some cognitive and social goals?

The topic I am to discuss with the professor is the pyschological adjustment of being diagnoised with diabetes. I am assuming the professor wants to be treated as a patient.
